Contra Costa Health Services Issues Statement On Thermometer Temperature Screening As An Infection Control Measure For COVID-19

The following statement is from Contra Costa Health Services:

The medical literature contains very little data regarding whether thermometer temperature screening “temperature screening” of asymptomatic persons can reduce the risk for spread of COVID-19.

Airport screening procedures during prior epidemics involving millions of screened persons including the SARS 2003, H1N1 2009, and Ebola epidemics have found no evidence that temperature screening reduced the risk of spread of infectious disease.

For example, despite mass airport screening protocols, no cases of Ebola were found and 4 cases were missed and entered into the US. There are no studies that address temperature screening for entry into facilities, but it is reasonable to expect similar experiences. The World Health Organization (WHO) has noted that temperature screening alone, at exit or entry, is not an effective way to stop international spread.

The medical literature on this topic is rapidly expanding and the prevalence of disease is fluctuating, so the discussion below may be subject to change if conditions change.

Despite the lack of known efficacy, temperature screening has become commonplace in the community as a surrogate test for infection with COVID-19. Some employees are being asked to check and record their own temperatures, and some are tested by the employer with devices such as temporal artery thermometers.

Employers running such screening programs need to know their drawbacks. One such drawback includes the reality that a substantial percentage of COVID-19 infected [and infectious] persons do not have a fever and febrile persons can mask their fever with antipyretics. A screened afebrile person may therefore give others a false sense of security. Another weakness is the often poor correlation between a screened person’s skin temperature and core temperature, which can be problematic for employers. For example, hot or cold weather can cause marked variation in the skin temperature but not the core temperature. Also problematic is the lack of sensitivity of currently available screening thermometers to detect core temperature elevation, which has led medical authorities to question their efficacy. Another issue is that characteristic facial flushing and elevated skin temperatures are often not found in the early phases of other known viral infections, which may reduce the ability to detect early viral infection by skin thermometer reading. In addition, the act of screening persons may increase the infection risk of the screener.

The screener may have to reduce recommended physical distancing protocols to get close enough to measure the screened person’s temperature. Some thermal camera systems have been developed which may avoid this last concern but these may not be practical or validated for facility use.

Employers should note that the Center for Disease Control [CDC] has published explicit guidelines for temperature screening for many workplaces. Some workplaces may also have specific society recommendations for temperature screening (i.e. skilled nursing facilities). If performed, temperature screening should be part of a larger education and pre-work screening effort which should include screening for common symptoms of COVID-19 and reminders of the importance of physical distancing, proper hand hygiene, and face coverings. Those who are directed to stay away from work should have explicit instructions, and employee information needs to be recorded and kept confidential. Procedures for dealing with screened persons who refuse to have their temperature taken should be detailed.

Facilities that have existing temperature screening programs in settings that the CDC has classified as ”optional” may consider discontinuing temperature screening and replacement with symptom screening. Some employers may want to continue to perform temperature screening in these settings due to local health concerns. Paid sick leave may reduce the chances that a worker will attempt to come to work with COVID-19 infection.

In summary, temperature screening as a means of COVID-19 detection is very common in the community but there has been scant evidence in the medical literature of its efficacy.

This situation is highly fluid, so employers are encouraged to first seek the most recent CDC guidance or specific society guidance on starting and running such programs. If the CDC has determined that temperature screening should be optional, Contra Costa Health Services generally does not recommend temperature screening. However, some facilities may wish to perform temperature screenings due to overriding local health concerns.

This coronavirus thing must really be running out of steam if they have the time to write such a long statement that says pretty much to follow what the CDC says.

If the county is looking to cut some jobs they should look at the Health Department.

It’s like everything else that has been done during this pandemic: its merely an attempt to try and stop the spread. Its not perfect, but there has been no one sure way to stop the spread (shelter in place not withstanding). Medical professionals continue to do their best, and all we can do is kind of roll with it. Gripe all you want about temp checks and wearing a mask, but this thing isn’t going away anytime soon and we just have to keep trying different things until a cure is found.

Stopping the spread was never the plan as that is impossible. The plan was to slow the spread so the infected would not overburden the hospitals and medical staff. That was achieved, and in fact, this past Sunday marked a 90% drop in deaths from a high set in April. Put another way, 2,693 people died from corona on April 21st, compared to 267 deaths on June 21st.

Seems significant enough for every news outlet to report, but the majority of the media continue to peddle fear and misinformation. Perhaps it’s why you believe all the continuing nonsense is an attempt to stop the spread, and we should just roll with it.

Finally, if you’re waiting for a cure, don’t hold your breath. There is no cure for the flu, AIDS, etc., etc., etc.

Looks like the ‘settled science’ has become unsettled again. If you wait long enough, you will get yet another ‘correction’ from our Fearless Leaders.

Meanwhile, businesses continue to close (permanently), people aren’t getting chemotherapy (Lancet says over 20% of people currently suffering from cancer stopped their treatments), new cases aren’t being diagnosed for cancer, heart disease, diabetes, etc…And our “Health Department” experts can’t quantify the harm caused by this Cower-in-Place policy which resembles a social experiment from the mind of Josef Mengele.

People may find out that they don’t really need treatment for ” … cancer, heart disease, diabetes …”

That is too bad. Grow up and face reality. Your comparison to Nazi Germany is twisted. If we did nothing to keep the virus from spreading would that make you happy? This is serious and we must be careful. What would your solution be? Ignore and think this is a hoax? Good luck with that idea.

Don’t want to alarm anybody but I work in San Francisco and ride BART. My job temperature tests everyday but someone still tested positive. Tomorrow I’m getting tested. I should find out the results by Friday. The only places I’ve been in the last week are a Safeway in Concord and one in Walnut Creek.

There’s ten minutes I’ll never get back. Seems like they could have just said “We’re clueless …” and saved everyone the time. Amazes me the people, including the Governor, who absolutely must comment on every little thing which they appear to know absolutely nothing about, and keep the issues as confused as they possibly can.

Oh, you are such a hero. Have you considered it might be a way to get an instant but inaccurate test? If you have a temperature you may want to get tested further.

I had no idea grocery stores were taking temperatures too. You would think they would be more concerned about making money. And they wonder why most of us have Covid-19 fatigue.

Based on my neighborhood experiences, I would say that Contra Costa Health Employees don’t care one iota about mask wearing and social distancing. Twice in the past 3 weeks, I’ve noticed one neighbor, an MD in the CoCo County Hospital emergency room, going out for a jog with a non family person (2 different people), no social distancing, not masks. She has also congregated with another neighbor, kids messing with the neighbor, she and he chatting with no masks and no social distancing. All the while her personal Facebook page states how stressed she is about COVID-19 and the dangers of it. But get her home, she is a cesspool of danger.

These county health employees that make large six figures, including benefits, are despicable when they act as my neighbor has. And yep, I sured as yeck yelled at her to quit congregating, and use social distancing and masks. Her response? I was ignored. Thank you for nothing, Ms. Estela, MD.

I will not let anyone check my temperature who is not a nurse or doctor. Temperature checks do not work. Masks don’t work. They admit it right in our face.

@Realust – what do you mean masks don’t work? 2 people wearing masks and social distancing is the best way to stop the spread. But of course I’m sure you don’t want to be “inconvenienced.” You must be listening to the guy who doesn’t believe in testing.

Are temperature checks perfect? No. Are they quick, painless, and will stop a portion of sick people at the door? Yes.

I would be more compelled to go to a grocery store requiring checks. It is for my own objective benefit that everyone gets scanned at the door, it makes it safer for me to shop.

How many of you have an oximeter? They’re very inexpensive and will tell you about your oxygen consumption. It is one of the most key things they look at first. My sp02 rate is usually 97 or 98 which is optimum.

Captain, thank you very much. I know someone who could use one of those things. It looks like some kind of light pulse estimates the oxygen content.
